Use this form to notify the Director of Consumer Affairs Victoria of changes to your funeral provider registration details.
Before you start
You will need to provide the following mandatory information for any change of:
- individual involved such as the legal owner or manager
- full name
- date of birth
- contact email address and phone number
- company or organisation
- full name
- any relevant registration numbers. This may be the ACN of a company, Incorporated Association registration number, ABN, etc.
